Need UX design for a modal - Upwork

Need a modal with 2 screens designed for entertainment site.
Attached are 2 drawings of the content of the modal and a general example of a modal.

This is 1st modal screen the user will see first. I've numbered the different sections on the sketch and provided clearer copy.

Should be created in Photoshop or any of Adobe CC products.
